I hereby give AsisImport s.r.o., Ident. No. 27203093, based in Holoubkovská 698, Praha 15 - Horní Měcholupy, zip code 109 00 (hereinafter referred to as the "Company"), explicit consent to the processing of my personal data communicated to the Company in a written form, by electronic communication, by telephone or in person in connection with the ordering of goods and services or, as the case may be, which the Company has legitimately obtained in another way (hereinafter also "personal data" only). I grant this consent to the Company for the purpose of concluding the Purchase Contract (s) for the goods supplied by the Company and ensuring the transport of these goods to the destination, respectively taking over, as well as for fulfilling my obligations towards the Company, including the recovery of the Company's claims against me.

I acknowledge that my personal data may be made available to employees of the Company, processors with whom the Company has concluded an agreement on the processing of my personal data, to third parties through whom the Company carries out its services, or to other persons authorized to process personal data.

I confirm that I agree that the Company is authorized to record and archive any communication with me and use it if necessary. I also agree that the Company shall acquire and process copies of my identity papers and other possible documents provided by me to the Company.

The Company is entitled, if necessary, to disclose my personal data to other entities with whom the Company cooperates in realizing the subject of the purchase contract, in the course of its business and in recovering receivables under concluded purchase contracts and any other agreements resulting from the subject of performance. These include, in particular, lawyers, tax advisors, accountants and auditors, postal, transport and telephone service providers, another persons providing services to the Company and other persons (eg supervisors, courts, bailiffs, auctioneers, etc.). The purpose of the data provided is the realization of the subject of the purchase contract (s), recovery of the receivables resulting from the fulfillment of these purchase contracts, as well as the realization of the Company's object of business and the fulfillment of the legal obligations and the protection of the rights of the administrator.

In case I provide to the Company a mobile number for a prepaid service or a fixed connection telephone number, I expressly declare that I am a participant of this telephone number / numbers.

I agree that my personal data will be used for the purpose

1. of creating and sending offers of trades and services of the Company and third parties, including through electronic means or via SMS,
2. of my participation in loyalty programs,
3. of selecting clients for creating product and service offers (especially in the form of direct mail campaigns) and for evaluating them,
4. of the Company´s marketing surveys.

I further agree that the Company will pass on my personal data to an economic subject affiliated through the statutory body and person of the partner which is WINE PLACE s.r.o.

I agree to grant this consent to the Company for an indefinite period of time, knowing that I can withdraw consent at any time and ask the Company to delete my personal data from all the databases where it is kept and archived, only if I have fully repaid all my obligations towards the Company and the Company will not record any arrears resulting from or related to the Purchase Contract (s) I had entered into with the Company. I may make a withdrawal of my consent in a written form through a notification delivered to the Company's address, electronically via e-mail This email address is being protected from spambots. You need JavaScript enabled to view it. or the relevant act in the client section on the website www.ait-praha.cz. In the same way, I may at any time withdraw consent to marketing purposes under the preceding paragraph.

Consent to the making and processing of copies of my identity papers and other possible documents under the third paragraph of this consent may be withdrawn only if the Company is not a liable entity within the meaning of Act No. 253/2008 Coll., on certain Measures against the Legalization of Proceeds from Crime and Financing of Terrorism, as amended.

I confirm that I note,

1.  that the Company is authorized or obliged to archive my personal data in order to protect the rights of the data controller and to fulfill his legal obligations (in particular obligations under Act No. 253/2008 Coll., on certain Measures against the Legalization of Proceeds from Crime and Financing of Terrorism, and Act No. 499/2004 Coll., on Archiving and File Service and on the amendment of some acts.)
2.  that for the purpose of fulfilling the Purchase Contract (s) the provision of certain personal data is mandatory in accordance with § 11 of Act No. 101/2000 Coll. on the Protection of Personal Data and on Amendments to Certain Acts, as amended. These mandatory personal information includes the name or business name, company ID / VAT number (if it is a natural person), billing address, shipping address, e-mail, telephone number. These data are strictly necessary for the performance of the Purchase Contract (s) and for the delivery of the object of performance to the place of destination. Other data, if required by the Company, for example for marketing purposes, are optional.
3. that, in accordance with § 12 of Act No. 101/2000 Coll. on the Protection of Personal Data and on the Amendment of Certain Acts, as amended, I may ask the Company for information about the processing of my personal data and that the Company is obliged to provide me with this information without delay. At the same time, I acknowledge that the Company has a right for an adequate reimbursement not exceeding the costs necessary to provide the information, and that this obligation may also be fulfilled by the processor for the Company.
4. that the Company provides personal data to processors primarily for the purposes of delivering the subject of performance, archiving, accounting, debt recovery, etc., in particular to ATTOP, spol. s r.o. with registered office at Březiněveská 57, 250 64 Hovorčovice, PPL CZ s.r.o. with registered office at K Borovému 99, Jažlovice, 251 01 Říčany, JUDr. Ing. Jan Matys with registered office at Sokolovská 25, 186 00 Praha 8 and others.
5. that, according to § 21 of Act No. 101/2000 Coll., I am, in particular, entitled to ask the Company to give me the information about what personal data I am processing, for what purpose, for whom and in what manner, and if I believe that their processing is in conflict with the protection of my private and personal life or the law, if my personal data are inaccurate with regard to the purpose of their processing, I have the right to request
   clarification or removal of this status, in particular by blocking, correcting, supplementing or disposing of personal data. I also have the right to turn to the Personal Data Protection Office in this case.
